Navigation (CAN)
Team
The CAN Team, in collaboration with 211 Sacramento and Sacramento Steps Forward, serves as an outreach resource for individuals experiencing homelessness. This team engages with those who have sought support through 211, ensuring connections to long-term case management services and/or shelter options. Recognizing the challenges of navigating resources while unhoused, the CAN Team bridges gaps and offers meaningful support.
Referrals
Referrals to North A Street can be accessed by calling 2-1-1 and dialing 8.
Requirements
To qualify for a CAN referral, you must be experiencing homelessness, currently seeking shelter, and not connected to any long-term supportive services.
